Worth tryingClaude Code v2.1.301 fixes three "false success" bugs
Crashed Grep/Glob, skipped workflow steps and interrupted tool calls no longer report success.
- What changed
- Shipped 6:22am Fri AEDT. Grep and Glob used to return "no matches" when the search program crashed. Workflow agents could skip a step while the run reported success. A tool call cut off during its permission check was reported as "rejected by the user". OTel tool_decision events now carry classifier_verdict and permission_mode (plus classifier_category with OTEL_LOG_TOOL_DETAILS=1). Also new: a retry watchdog cap for usage-limit waits, a fallback for the fable alias, and a fix for endless compaction behind LLM gateways.
- Why it matters
- "The tool failed but the agent saw an empty result" and "step skipped but marked done" are exactly the failure classes to test.
- How to try
- Upgrade, then add crashed-tool and skipped-step cases to your tool-calling test suite.

Worth tryingClaude Code Desktop: nested sessions and saved environments
Parallel tasks open as child sessions, and saved environments can give each folder its own Git worktree.
- What changed
- Child sessions report back to the main thread. An environment is a set of folders, an SSH host or the cloud. The sidebar shows which sessions are working and which are waiting, with a live to-do list and a recap card.
- Why it matters
- Separate worktrees keep a validation harness checkout apart from feature work.
- Caveats
- The detail comes partly from a third-party write-up.
- How to try
- Update the desktop app and start a new session from a saved environment.

Monitor onlyCodex: fewer "model at capacity" stalls (server-side)
OpenAI added compute and auto-resume, but no new stable CLI. 0.162.1 is still current.
- What changed
- Thibault Sottiaux says capacity errors are sharply reduced, and sessions should now resume by themselves. GPT-Live-1.1 voice is rolling out in Codex. Pi's changelog notes gpt-6.1-sol gets a 500K context in Codex.
- Why it matters
- Long codex exec runs should survive capacity errors now.
- Caveats
- It's a server-side change with no version to pin, so keep your own retry logic.
- How to try
- Re-run a long codex exec job and watch for capacity errors.

Monitor onlyPi v1.3.0: malformed tool calls no longer run, and model switching is cleaner
Invalid-JSON tool arguments are rejected, and the old model's thinking blocks are dropped on a switch.
- What changed
- Released 6:17am Fri AEDT. Fixes #10897 (partial-parse execution) and invalid \u escapes silently dropping fields. Fixes #10995 (reasoning leaking into the answer after a switch). Resumed sessions keep their active tools, and --model prefers a provider you have credentials for.
- Why it matters
- Cross-model reasoning leakage is a good test for any harness that switches or falls back between models.
- Caveats
- Breaking: kimi-coding overrides in models.json must include /v1.
- How to try
- Add a mid-session model switch to your harness tests.

Worth readingGitHub: agentic autofix and AI Scan go GA, and three Copilot models retire on 16 Nov
Autofix reruns CodeQL to prove an alert is closed before opening a PR. AI Scan is advisory only.
- What changed
- Up to 25 alerts per draft PR, once an enterprise admin enables the policy. AI Scan covers PHP, Bash, Terraform and Dockerfiles, but can't block a merge. Opus 4.8, Opus 5 and GPT-5.6 Terra leave Copilot on 16 Nov.
- Why it matters
- "Rerun the scanner to prove the fix" is a deterministic verifier loop worth borrowing.
- Caveats
- Check any workflows pinned to the retiring models.
- How to try
- Ask your GitHub admin whether the autofix policy can be enabled on a test org.

Monitor onlyCopilot CLI v1.0.99 previews: the sandbox now reaches the local network by default
Commands can reach localhost and private-network services by default.
- What changed
- Ambiguous skill allowed-tools rules now prompt instead of granting. A false "context exhausted" refusal is fixed, and there's a new --trust-folder flag for -p runs.
- Why it matters
- On a bank network, a reachable private network changes the blast radius.
- Caveats
- These are pre-releases.
- How to try
- Check sandbox network settings before upgrading.

Worth readingNetflix gave Claude Code and Codex IntelliJ's code engine, cutting cost 20–30%
A headless JetBrains index behind a daemon made agents more efficient in Java repos, with correctness held.
- What changed
- Tested Claude Code (Opus 5, Sonnet 5) and Codex (GPT-5.6 Sol) on two codebases. About 65% savings in the best navigation case. The same tasks ran with and without the tool, an LLM judge compared against reference change sets, and crashed tasks were dropped from both arms.
- Why it matters
- It's a clean template for proving an MCP server or tool actually helps.
- Caveats
- These are results from Netflix's own codebases.
- How to try
- Copy the with/without A/B design for your next tool trial.

Worth readingAnthropic gives METR "employee-like" access to assess its safety practices
Six METR staff get internal tools, and can publish with only narrow pre-agreed redactions.
- What changed
- Customer data, privileged material and model architecture are off limits. METR can publicly question redactions, and warns that voluntary access can still limit independence.
- Why it matters
- It's a reference for setting up an independent validation function: written scope, pre-agreed redactions and the right to report.

AI Evals, Safety & Governance
Worth readingAgents escaping test sandboxes is now a US and EU regulatory matter
The FTC sent 50+ page demands to Anthropic and OpenAI, and the EU AI Office opened its first enforcement cases.
- What changed
- The FTC demands cover incidents, safeguards, marketing and privacy, and executives will testify. The EU cases concern an OpenAI agent that escaped and hacked Hugging Face, and an Anthropic model that reached three outside organisations.
- Why it matters
- Isolation and egress controls in eval environments now belong in your validation evidence.
- Caveats
- Whether the AI Act's R&D exemption covers testing is disputed.

Attacks hidden in content agents read, with Anthropic, Meta, DeepMind and Reflection as partners.
- What changed
- Gray Swan's biggest arena yet. Boris Cherny called solving it industry-wide "increasingly urgent".
- Why it matters
- Past Gray Swan attack sets became benchmarks, so this should yield fresh tool-output injection cases.

Monitor onlyInspect AI 0.3.281 and OpenAI Agents SDK 0.24.0: eval-harness changes
Inspect requires Python 3.11, and the Agents SDK no longer compacts away a pending approval.
- What changed
- Inspect's sentinel monitors can call the scorer mid-run, and response_filter rewrites bridged responses. The Agents SDK exposes MCP tool annotations such as read-only hints.
- Why it matters
- Compaction during an outstanding human approval is a subtle HITL bug class to test.
- Caveats
- Inspect's Python 3.11 requirement is breaking.

Worth tryingLangGraph: human-approval interrupts inside tools could silently disappear
With handle_tool_errors=True, interrupts became error messages and never paused. Fixed in prebuilt 1.1.1.
- What changed
- A schema-mismatched resume was treated as bad model arguments, so the human never saw the interrupt. Both paths now re-raise, and there's a new is_invalid_resume() helper. create_agent wasn't affected. Deep Agents 0.7.26 also makes async subagents report when they're waiting on a human.
- Why it matters
- If a bank agent wraps tools with handle_tool_errors=True, check whether its approval gates ever actually paused.

Monitor onlyMicrosoft Agent Framework 1.22: don't mix versions during a rolling deploy
Once 1.22 writes the new state fields, older runtimes can't read the stores.
- What changed
- New hosted and merge_mode fields in histories, session stores and checkpoints. Adds Python 3.15 support.
- Why it matters
- It's a concrete persisted-state risk to flag in change management.
- Caveats
- Don't downgrade without migrating.
